Hello Parents,

The registration for Fall Sports opens on August 1st! Saint Michael School Sports are open to 5th - 8th grade students. Fall Sports are as follows: boys and girls soccer, co-ed volleyball, boys and girls cross country.

Registration is completed on-line at League Toolbox. This is the website that the EAA uses for all sports. If your child has been on a team in the past few years, you already have an account on League toolbox and only new participants will need to set up an account once you enter the site

Once on the site you will be directed to registration.  Please include your email in your accounts because this is how both I and the EAA will contact you.  

Each student is required to have 5 forms for a complete registration. 

1. EAA Medical treatment and liability release form (may be electronic on the site) 

2. Student concussion certificate

3. Parent concussion certificate 

4. Parent-Athlete concussion information form

5. Pre-participation head injury concussion form

All of the forms must be filled in completely including signatures and dated or they will be deleted by the League and your child will be ineligible to play until they are filled out correctly. The EAA liability form was supposed to be put online so hopefully you will be able to complete it electronically. The other forms are available at the website for you to download, fill out, and re-upload to the website. If you do not have a scanner you can use your phone to take a picture of the form and upload them to League Toolbox.  I have also attached them here if you have any difficulty.

Each student/athlete and parent must both separately complete a concussion in sports training course at the National Federation of High School Sports website  http://NFHSlearn.com

If you have done this in the past you already have an account. Any new athletes will have to make an account, the students can use their Saint Michael School email for this site.  Students and parents take the same free course but at different times and must have a separate certificate for each with both their first and last names.  At the completion of the course you will receive a certificate.  Save them to your computer so you can upload them to League toolbox.

I would also like each family to let me know which sport your child(ren) will be participating in this Fall by completing the Google form here and submitting it to me. Fall sports teams
